Atticus Health is seeking a compassionate and adaptable Home Care Partner / Practice Nurse to join our team in a full-time role supporting the Palm Lake Resort Paynesville community and surrounding home care clients.
This unique role combines face-to-face nursing support with facilitating telehealth consultations when the GP is not onsite. The GP will provide face-to-face consultations within the community 2 days per week.
About the Role
- Support residents within the Palm Lake Resort Paynesville community
- Facilitate telehealth consultations between patients and the GP
- Assist with face-to-face GP consultations when the GP is onsite
- Provide clinical nursing care including observations, wound care, health assessments, chronic disease management, and care coordination
- Support home care clients within the local area
- Maintain accurate clinical documentation and communication with the healthcare team
- Deliver compassionate, patient-centred care
About You
- Current AHPRA registration as a Registered Nurse or Enrolled Nurse
- Experience in general practice or home care preferred
- Strong communication and organisational skills
- Comfortable using technology and telehealth systems
- Ability to work independently and collaboratively
- Current driver’s licence and reliable vehicle preferred
