Home Care Manager – Mornington Peninsula

Are you a compassionate and experienced professional with a passion for delivering high-quality home care services? We are seeking a dedicated Home Care Manager to join our team and lead the coordination and delivery of care services to our clients in the Mornington Peninsula area.

About the Role

As the Home Care Manager, you’ll be responsible for leading and overseeing daily operations, ensuring top-tier service delivery, and supporting your care team to deliver person-centred care that promotes independence and quality of life.

Key Responsibilities:

  • Leading and mentoring a team of care workers
  • Developing and reviewing client-centred care plans
  • Ensuring compliance with aged care standards and regulations
  • Conducting regular client assessments and service reviews
  • Liaising with healthcare professionals, clients, and families
  • Managing care budgets and service agreements
  • Promoting continuous improvement and quality assurance

What We’re Looking For

  • Registered Nurse (RN) or Enrolled Nurse (EN) with current registration
  • Proven leadership and organisational skills
  • Excellent interpersonal and communication abilities
  • Strong knowledge of aged care regulations and home care best practice
  • Proficiency in using electronic health records or care management systems

     

We believe home care is personal and happens in person.
This role is not suitable for those seeking remote or work-from-home arrangements.
