Atticus Health is seeking a compassionate and motivated Care Coordinator (Registered Nurse) to join our team. This unique role combines hands-on nursing with care management, supporting older adults to live healthier, more meaningful lives.
About the Role
- Location: Based in our Hardware Lane clinic (CBD) with outreach across the city and surrounding suburbs.
- Split Role: 50% clinical nursing within the clinic, 50% care coordination in the community.
- Focus: Partnering with people over 50, both in clinic and at their homes, to improve their quality of life.
Key Responsibilities
- Provide nursing support, including clinical assessments and care delivery in the Hardware Lane clinic.
- Visit patients in their homes, building trusted relationships and supporting them to live well and independently.
- Act as a Care Manager for patients aged 50+, coordinating care plans and services tailored to individual needs.
- Collaborate with GPs, allied health, and community providers to ensure seamless, person-centred care.
- Support patients in navigating the health system, connecting them with relevant services and resources.
- Maintain accurate records and documentation in line with professional standards.
About You
- Registered Nurse (AHPRA) – Essential.
- A genuine “people person” – warm, empathetic and able to connect meaningfully with people over 50.
- Strong interest in holistic, person-centred care and improving life quality for older Australians.
- Excellent organisational and communication skills.
- Current driver’s licence and willingness to travel locally to patients’ homes.
