There are way too many silos between health care and social services in Australia.
Work in a very meaningful role, breaking down those silos, integrating health care and home care.
Your job will be to work primarily as a home care manager, establishing client plans, allowing the elderly live independently at home. This will be around Melbourne City and the inner Bayside suburbs of Melbourne.
Importantly, you will also be responsible for creating a connection between home care workers, clients, and their usual GP.
Preferred experience
- A Registered Nurse (RN) or Enrolled Nurse (EN) with current AHPRA registration
- General Practice Nursing with an understanding of Medicare and how a medical clinic runs
- Home Care experience or other community nursing roles
Note that we are willing to consider applicants with none of the above experience provided they are the right character fit and highly motivated to get involved.
